The first champion was crowned when Kalidah, a heavyweight defeated the reigning cruiserweight champion Sangre de Pollo in the final stage of a Scramble Cage match on 6/3/2024 in Toledo, Ohio. As Kalidah was ineligible to become cruiserweight champion, PMLL elected to crown Kalidah as its first heavyweight champion while vacating the cruiserweight title.
